An employee was stationed on the winder end of a winding machine and tasked with observing the product (roll of paper poly/film) being rolled. The employee had his left hand crushed between the floating shaft (holds roll on machine) and ejector roll bar, resulting in the ripping/amputation of the tips of the middle finger and ring finger by the machine.
