An employee had been inspecting an HVAC unit on the roof of a building. He then climbed down the first section of a fixed ladder from the roof and was standing on a metal platform before climbing down the second section of the ladder. He fell through the opening in the platform where the second section of the ladder starts, and landed on the ladder's base, which is a raised metal platform. He sustained fractures to the left arm and right ankle.
