Final narrative
An employee was moving a steel containment grate. The plant had a water main break and the maintenance employee had to move the containment grate to get a pump into the pit. The steel grating slipped out of his hands and landed on the right index fingertip, pinching it between the concrete floor and bottom of the grating. The employee sustained a fractured finger and an amputation of soft tissue from the finger.
