An employee was working on the second level of the building. While he was walking to go on break he stepped on the corner of a floor covering that was covering an opening in the floor. The covering had shifted and the screws that were holding the covering to the concrete broke free. The floor covering flipped up and the employee fell through the opening onto the ground below. The employee fractured his nose and both elbows.
