On January 6, 2021, an employee was installing a tarp on a semi-trailer while standing on the tarp rack platform next to the trailer. As he was descending the platform, he slipped and fell between the trailer and the tarp about 12 feet to the ground. He suffered a broken left ankle and was admitted to the hospital.
