An employee was working at a roll former, feeding a coil of steel through the rollers to form a ring. The steel became misaligned at the second of three forming rollers. The employee was aligning the piece when the roller caught his right glove and drew in his index, middle, and ring fingers. He suffered a fracture to the index finger and crushing injuries to the middle and ring fingertips, with amputations at the fingertips. The machine was guarded at the time.
