Final narrative
An employee accessed the top of a piece of equipment from a platform and had been removing a damaged motor from the equipment. The employee fell down the side of the 8-foot high piece of equipment and struck a rail and the concrete floor. The employee was hospitalized with three broken ribs and a contusion on the side of the head.

A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground.
