Final narrative
An employee was working on an automatic cutting line. Noticing that the chain had slipped off the gear that moved the material down the line, the employee accessed the machine area and tried to place the chain back on the gear. The employee's left hand became entangled in the chain, and the employee suffered amputation of the left ring finger up to the knuckle and lacerations to the left middle and little fingers. The machine was not locked out/tagged out at the time.
