Final narrative
An employee was performing routine maintenance on the discharge pipe attached to the chipper. The employee climbed onto the equipment using a ladder and was lowered to the ground using a bucket with a two-foot rim. While exiting the bucket, the employee tripped and fell head first onto the concrete ground. The employee sustained a skull fracture.
