Final narrative
A mechanic was working with a loader operator to replace the sheared pin of a quick connect system on a front-end loader. The mechanic was manually aligning the cylinder end with the attachment connection shaft when the quick connect system was actuated. The mechanic's index and middle fingers were caught between the cylinder end and the attachment connection shaft. The middle finger was amputated above the distal interphalangeal joint.
