Final narrative
An employee was putting a motor and plate back on a starch pump. As he balanced the motor between his forearms and guided the adjustment bolts into the plate holes, the motor slipped out of his arms. His left little finger was pinched between a nut, a washer, and the plate, causing an amputation to the finger.
