Two employees were installing a fuel tank on a wheeled loader. They were using a manual transmission jack to lift the fuel tank into place to secure it to the frame of the machine. While one employee was operating the jack to line up the third hole, the injured employee was feeling the bolt hole to ensure it was lined up. The tank slipped, causing the injured employee s left middle finger to become caught between the bolt hole of the fuel tank and the bolt hole of the frame of the machine. The employee sustained a fingertip amputation.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
