An employee was forming parts on a brake press. The employee was recreating and bending a long piece of metal that was on the bottom die and resting on the mat on the floor. When the die came down, the piece of metal became caught on the mat. As he reached down to remove it from the mat, he lost his balance and his left hand went into the press that then cycled. The press was stuck in the down position, resulting in a crush injury to the employee's left hand that required medical amputation of the little and ring fingers.
