On May 3, 2023, an employee was holding rigging hooks in position to lift a rebar coil and spool for an automatic rebar bender. The hook slipped in the employee's hand while the sling was being tensioned with a crane. The employee's right index finger was caught in a pinch point between the hook and the tube steel used for lifting the rebar and coil spool assembly. They suffered an amputation to their right index finger.
