An employee was un-pinning a part from a shipping crate that would be lifted out of the crate by an overhead crane. The pin consisted of both a pin and a sleeve. The pin was removed, however the sleeve remained in place. The employee then used her finger to help push out the pin sleeve. As the sleeve cleared the crate, the part abruptly moved upward due to the upward tension and caused a shearing impact to the employee's left ring finger. The employee sustained a fingertip amputation with an open fracture.

An employee was repairing the suspension of a semi-trailer. He was installing an air bag to the suspension when his left ring finger was crushed between the air bag and the suspension mounting beam. The employee was hospitalized.

An employee was helping a crew move a section of a grain conveyor that weighed 600-lbs into the proper configuration. The piece of equipment was sitting on a transport cart. While turning the cart to align it, the piece of equipment shifted and the employee's right middle finger was pinched in a small gap between the equipment and the cart. The employee pulled his gloved hand away and suffered a fingertip amputation.

On May 16, 2025, an employee was performing iron work. He was sliding a 3/4" x 2' x 4' bent plate across another plate to position it for installation. The bent plate crushed his left middle finger between the two steel plates. The distal bone was fractured and required a revision amputation.
