Two employees were using pipe wrenches to loosen a rotating shaft on a pump. One of the wrenches attached to the shaft rotated along with the shaft and struck an employee's head. The employee sustained a concussion.

The employee pulled his gloved hand away and suffered a fingertip amputation.
