An employee was starting up a calender machine. The startup procedure required the employee to thread a string on a plastic pole to feed through the machine. While the employee was threading the pole, the machine caught their left thumb. While the employee was trying to extricate their thumb, the machine pulled in their right arm. They suffered a fracture to the left thumb, as well as a crushing injury and skin removal to the right arm. The employee was hospitalized, requiring surgery.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.
