Final narrative
An employee was operating the dock lift using the button on the power box located between dock bay stalls. As she pushed the button to lift the ramp, some all-purpose containers (APCs) also began to rise. As she turned around, an APC became unwedged and rolled toward her. She fell backward off the dock platform and landed on her feet. The employee sustained injury to her right leg and left knee.

The pipe jack/steel tube used to raise the pipe slipped, causing the steel tube to strike the employee's face. The impact caused the employee to fall off the ladder and sustain fractures to their left tibia and fibula.
