At 1:49 AM on January 11, 2025, an employee was adjusting the rear chuck on a CNC machine into a comfortable angle to use a speed wrench while loading/securing a stabilizer in the machine. The employee was holding the base of the speed wrench with their left hand. Once the rear chuck was in position, the wrench shifted, causing the chuck to roll. The employee's left little finger was caught between the wrench and the edge of the rear chuck guard, and the fingertip was partially amputated.
