An employee was cleaning the floor with degreaser. They lost balance and reached out to grab the side of the fryer. This caused the fryer to tip over in the direction of the falling employee. The oil poured out from the fryer oil well and onto the employee and surrounding floor. The hot oil covered the employee's body. The employee suffered second- and third-degree burns to over 50 percent of their body.

A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.
