On April 19, 2025, an employee was performing preventative maintenance by spraying lubricant onto the chain drive for a metal scanner conveyor. The employee noticed some overspray on the adjacent frame of the equipment and took a rag to wipe off the lubricant. The rag became caught between the chain and a sprocket, which pulled the employee's right hand into a nip point. The employee's right index fingertip was amputated. The machine was running and was not guarded at the time of the incident.
