A crew was aligning a pad-mounted transformer with two 3-inch rigid metal conduits that protruded through the pad. The transformer needed slight repositioning, and an employee stepped in to help guide the adjustment. The transformer base shifted, and a finger on the employee s right hand was caught between the cabinet base and the conduit top. The fingertip was amputated at the distal knuckle.

As he walked behind the parked forklift, it started rolling and the employee was struck by the forklift and pinned under a tire. On August 15th, 2025, an employee was aligning/connecting a coupler to the generator of a wind turbine. The coupler was secured in place by a strap that was connected to the turbine's chain hoist and a bolt. While lining up the remaining holes, the coupler rotated while the employee's right index finger was in one of the bolt holes, causing a partial amputation through the phalanx.
