Final narrative
A crawler crane was being used to move steel columns from one laydown area to another at ground level. Two employees rigged shake-out hooks to the ends of a column at ground level. The crane began to raise the load. The rigging became taut and the load rolled forward as it found its center of gravity. One of the two employees had their left hand on it, and the tip of their left middle finger was pinched between the wire rope rigging and the column. The fingertip was amputated, including the nail bed.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
