An employee was working to install a pipe coupling on a section of steel pipe. The pipe became dislodged from its temporary support structure. As the employee went to gain control of the pipe, his right little finger was caught and amputated near the knuckle in a pinch point created by the pipe.

During crane assembly, an employee's left middle finger was caught in a bridle pin while trying to move counterweights, resulting in a fingertip amputation.

D.L. Gasser Construction, Inc.

An employee was spotting the dust auger as it was lowered onto the trailer so that it would not contact a piece of equipment already on the trailer and pinch electrical cords. As the auger was being moved into place horizontally, his thumb was caught between two brackets, one on the auger and one on a piece of equipment already on the trailer. The employee sustained a partial amputation to the left thumb.

An employee was working in the ceiling on an HVAC unit. While switching out a part, an evaporator coil struck the employee in the chest, leading to multiple fractured ribs on their right side. The employee was hospitalized.

An employee was working on the cylinder of a railroad truck to repair a hydraulic leak. The blade was being kept in place by a 4-by-4-inch piece of wood while he pulled a lever to force the blade flush against the block to secure it for the inspection. The employee sustained an amputation to the left thumb tip.
