An employee was on a ladder, cutting a piece of flex ducting. The ducting came loose. The employee lost his balance and fell, the rungs of the ladder catching his feet. Both his left shoulder and his head hit the concrete floor. He suffered a concussion and a broken shoulder blade and was hospitalized.
