What happened
The team leader and three other team members locked out the shredder for safety and entered the mill and attempted to remove the debris using a crane hoist and chain. After initial attempts failed they had to re-enter the mill and reposition the chain. During the second removal attempt the material slipped striking the lead employee 's left leg.
