What happened
Employee was operating a forklift moving construction materials. During this operation the forklift passed by a pallet containing construction equipment and materials. A large prybar was among those items. The length of the bar was greater than the width pallet causing the end of the bar to extend beyond the edge of the pallet. As a result when forklift passed the pallet the forward tire caught the bar flipping it into and through the forklift operator 's cab. The employee sustained a laceration to his forehead as a result.
