[REDACTED] worker notified his supervisor [REDACTED] that he somehow got debris in L eye again (see previous [REDACTED] injury report) during work hours on [REDACTED] or morning of [REDACTED] while drilling metal parts at his work station bench. Upon investigation of this injury and previous injury it was found that debris introduced to left eye could have occurred during donning of welding hood or hardhat which had been improperly laid upside-down on bench during grinding and or drilling (gathering debris) or while removing face shield which may have accumulated debris during grinding.
