Before the incident
Operator was foaming underneath HCVs 17 & 18.
What happened
Operator went to pick up foaming hose and while grabbing the foaming hose the handle that allows chemical to release got caught on a pipe and was then turned on. When the foaming hose turned on the sudden release of chemicals caused the operator to lose control of the foaming hose and spay himself in the face chemical got underneath the face shield. Operator was wearing all necessary PPE required for this task and used the eyewash station for the recommended amount of time. Eyewash stations were all above required temperature so operator had to use the process water hose on Vat Deck instead.
