Before the incident
EE was making her way to the breakroom for our morning standup meeting As she was heading to the breakroom she decided to grab her hairnet first which is placed inside a container near the Slicing Room 1 entry door Below the container is a floor mat and as she was stepping onto the floor mat it moved forward which was the cause for her slipping She hurt her right ankle and lower part of her back from the fall She stated that everything else is fine and that her pain level was a 6 out of 10 in her right ankle
