The Maintenance Manager was performing maintenance work.

While equipment was stopped and proper LOTO applied. He was climbing up on the clutch support assembly and motor belts working with chain hoist belts when suddenly lost his footing causing a 3 ' 4 ' ' fall into the motor basement. The momentum threw his body to the left side and hit his left shoulder against the clutch and floor causing an injury. He was taken to the hospital by ambulance in a resulting in humerus bone fracture.
