Before the incident
Operator was loading a bundle of small rod on the entrance table of [REDACTED] grinder. He was standing on a small platform in order to manually separate the rods and get them in a single layer. Since there were some shorter rods in the bundle the operator had to step off of the platform onto the ground between the platform and the table to reach these rods. The operator lost his footing (slipped) on the oil buildup on the floor causing him to fall into the table. The operator struck his back and ribs on the table causing some redness and discomfort.
