Material was hanging up and employee was prodding the material with a PVC pipe to free it. The material was freed and was captured by the grinder blade. This caused the slack in the material to tighten suddenly. The sudden tension put force on the PVC pipe and accelerated it towards the employee.

The employees right eyebrow was lacerated by the end of the pipe.
