What happened
[REDACTED] was unpacking formalin in the OR breakout room at [REDACTED]. The package was faced upside down. [REDACTED] unpacked it and the formalin spilled to the ground causing some of the containers to open and spill liquid on the ground as well. [REDACTED] used her bare hands to pick up the wet containers and the liquid made contact with her skin. She was advised to immediately stop handling the containers and wash her hands. [REDACTED] complied. There was no visible injury to [REDACTED] 's hands nor did she report any symptoms. [REDACTED] continued to work throughout the day with no issues.
