Before the incident
This patient had been really impulsive the [REDACTED]. Patient frequently setting off the bed alarm by putting his legs over the side rail scooting his legs and but to the corner of the bed sliding his legs over the bottom of the bed. At night patient provides very little assistance to turn from side to side or scoot up higher in bed. After repositioning this patient multiple times [REDACTED] and preventing him from falling I noticed a little muscle soreness when I reposition higher in bed standing on the right side of his bed. The pain gets worse with any boosting from the left shoulder up or when I extend my arm.
