What happened
EE was working with a co-worker throwing cardboard into the baler compactor and stated that the machine started and then stopped so he went to investigate why he did not de-energize the machine before proceeding. He walked around to the opposite side and stuck his finger in the hole where the sensor is and cleared cardboard which caused the compactor to engage and amputated 2 3rds of his left index finger.
