Before the incident

EE was removing the [REDACTED] base from the foaming fixture when the incident occurred.

What happened

EE had removed a foam ball from the vent hole which inadvertently removed a retaining clip from the pin securing the base unit to the foam fixture lid. The base unit could not be pulled as the lid was removed. The EE was working with a coworker to reinstall the pin retaining clip. The co-worker asked the EE to reach into the mold. The pin was reinstalled and the EE was holding the pin in place with both hands. The coworker lowered the overhead crane in which the foam fixture lid was attached to back in place to allow for the retaining clip to be reinstalled. Inadvertently the EE 's left hand - fifth digit and right hand-wrist were crunched as they were caught between the lid and the base of the foam fixture. The weight of the lid caused the crushing injury.
