Before the incident
RSE never reported the incident. Director of Operations overheard RSE discussing with another RSE. RSE was questioned about it and RSE stated they were fine. Director of Operations informed Branch Manager of incident. Branch Manager submitted NOI and instructed RSE to call Axiom. Axiom instructed RSE to visit a clinic. Due to being out of town and a non driver RSE was picked up the following morning [REDACTED] and taken to the clinic. While in route to the clinic Branch Manager asked RSE if they were aware of the reporting procedure to which the RSE said they were not. Branch Manager reviewed reporting procedure to RSE. Branch Manager asked RSE how the injury happened. RSE stated they were just stretching their leg from standing all day.
