Before the incident

Employee was using a 3 lb dead blow hammer to break down a wooden box.

What happened

Using the dead blow hammer to break the wooden box the employee swung the hammer and connected with the end of the box seperating a 2 x 6 piece of wood from the rest of the box the piece fell to the floor with a nail sticking up and the employee had lost his balance and stepped on to the piece of wood and the nail punctured his right foot
