What happened

Employee claims she slipped and fell on an oil spill. No management member was aware of any spill on the floor. In speaking with one employee who noticed the oil drip said it was very small and not that noticeable. She 's claiming she injured herself around [REDACTED] but did not mention the alleged incident until around [REDACTED]. She is also claiming to have taken a picture of her leg slipping in the oil drip. When management was notified of the alleged incident myself along with the SDOs went to the location of the alleged incident and the location of the incident there was a spot where a forklift imprint was there but no signs of footprint other than the one I made when i put my put in the exact same area. This employee was attempting to solicit statements from other employees indicating there was a spill and that she slipped. The employee proceeded to walk with a limp and was observed walking with and without a limp and at times limping on both legs.
