What happened

On the morning of [REDACTED] carrier informed me that the reason she called of the previous day is because she went to the doctor. She disclosed to me that the injury she sustains was on the job. I asked the carrier when did the accident occur she said [REDACTED]. She also stated [REDACTED] [REDACTED] she had another accident I asked once more” Did you inform any EAS?” She said “No”. Later I received a called from the carrier at [REDACTED] stated she suffered another accident. She stated the door on the residential building on [REDACTED] onto her foot while delivering. which caused her to fall face first on the st of the building. She sustain hip and knee injuries due to the fall. I advice her to call the ambulance on to the scene. When approaching the scene of the incident the carrier was nowhere to be found.
