Custodians were working cardboard off the dock area into the cardboard dumpster. One was tossing cardboard by hand from inside the dock area to the outside dumpster when after emptying a box of cardboard he took the empty pallet through the dock door and a strong wind caught it and blew it toward The injurd employee striking her on the top of the head. Statements were supplied a typewritten statement dated [REDACTED] with a handwritten notation At this current moment I am not seeking medical attention. I received a telephone call at [REDACTED] on [REDACTED] from injured employee advising me that she still had a headache from the incident and she advised that she needed to seek medical attention.
