Walking on the dock.
What happened
I had stepped off the forklift between dock door 14 and 15. I was going to turn off the lights on another forklift at door 13. I was walking towards the forklift and caught my foot on the dock plate. That caused me to start stumbling and I couldn 't catch my balance. I then fell hitting my head on the forks that were resting on the ground followed by my right shoulder striking the ground with my right knee striking the ground last.
