What happened
On [REDACTED] at approximately [REDACTED] I was loading OTR 's onto a 53 foot trailer when a box fell out of the top of the OTR and struck me on the back of my head and neck. I felt pain and I went and sat down for a while. I eventually finished loading the truck and came back to the plant. I took 4 ADVIL and reported the incident to 204B as soon as I arrived to the office.
