Unloading wire cage

On [REDACTED] Miami Logistics motor vehicle operator came to the office and stated that he injured himself at the [REDACTED] located at [REDACTED] on [REDACTED]. This accident was not reported to me until [REDACTED] the following day. I sent him to Concentra Urgent Care located at [REDACTED]. He stated that he was unload a wire cage loaded with mail off his truck and rolling onto the scissor lift. He states that the clerk was controlling the lift. As it started descending the wire cage rolled onto his left foot striking the second toe from the left on his left foot. Employee was wearing rain boots due to inclement weather and not wearing work shoes boots. He handed in his keys and vehicle and ended his tour He states that he did not feel the pain until he took off his boots when he got home. I was only able to get a photo of his foot showing a bruised toe. employee filled out forms [REDACTED] and [REDACTED].
