On [REDACTED] the employee came to me on my way out the door to inform me that her wrist was hurting after she tried to pick up a tray that was heavy. She immediately followed up with the fact that she doesn 't want to file a report or seek medical help. She just wanted to make me aware. [REDACTED] she claimed she wrote a statement and gave it to another supervisor to give to me but I have not seen such statement so I advised her to rewrite one. On [REDACTED] she came into the office to inform me that she went to her doctor regarding another issue with her hand and also mentioned her wrist hurting. The doctor told her it was sprained and gave her a brace to wear while working. And that 's when she decided she should report the incident. So on [REDACTED] she filed a report.
