LOADING MAIL

On [REDACTED] at [REDACTED] I received a call from Miami Logistics supervisor to quickly come to [REDACTED] Annex stall F with a first aid kit for Miami Logistics [REDACTED] driver. [REDACTED] driver was attempting to load a BMC onto his Postal [REDACTED] and while adjusting the BMC he slipped down the open area between the back of the truck’s cargo area and the dock plate. [REDACTED] driver 's left leg was split open in the middle. He claimed that because the dock plate was lifting he had to push the BMC harder and maneuver at an angle to get it into the truck. The only fault I noticed in my investigation was that he failed to pay attention to where he placed his foot. The issue with the dock should have been reported prior to him attempting to load a BMC. He could have also asked for assistance. [REDACTED] driver failed the safety protocols and was not paying attention to his foot placement into an open area.
