On [REDACTED] [REDACTED] at [REDACTED] I had delivered a package to the door at [REDACTED] While walking down the driveway bat to my truck I rolled my left ankle on the edge of the uneven driveway causing me to fall down on to the concrete. I ended up on the floor with a deep scrape to my right knee as well as some minor scrapes to my right palm left elbow and left knee. I contacted my supervisor immediately.
