What happened
On [REDACTED] employee ran PVS Route in [REDACTED] backed up on [REDACTED] of Dock. In order to get onto the Dock employee stepped up onto the back step of the 11-ton cargo truck backed into [REDACTED] and then the dock bumper of bay broke off the weld that was securing it to the wall and employee fell back on the ground in-between the 11-ton cargo truck and his 38-foot tractor trailer.
