What happened
On today [REDACTED] at approximately [REDACTED] I was walking into the employee entrance and slipped and fell at the entrance door. Injury to my right hip right wrist and shoulder. Management has not placed any anti slip mats at the entrances and was very slippery condition with the rain. This was at the automatic doors inside the first entrance gate.
